I had this problem some time ago, and it was fixed a few days ago. After the last update which fixed the lensflare being visible through walls bug, its back.

 

Lensflare dissappears when getting close to it.

Your lens flare entity was actually inside the ceiling light model, so at some distances the model was blocking all pixels in the occlusion culling render. I saved the map with one example light where I moved the lens flare down just a little so it's outside the model.

Yes, just make sure they have a little space to be outside the model so there is a clear line of sight. The occlusion query renders a 16x16 pixel square, so that is why it wasn't exactly the same result at all distances.
